Identifying and Recognising Frailty

This webinar will focus on understanding frailty and how to identify frailty using the Rockwood Clinical Frailty Score (CFS).
Carol is a Community Specialist Paramedic based in Knutsford which from late last year also includes Macclesfield. Her role involves working and collaborating with other agencies, in particular, GPs, Community Matrons, CCGs, Care Homes and local schemes for example frailty teams. Due to the demographics within my areas, this led to her developing her role to support older people in particular. She attended an Allied Health Professionals event for identifying frailty; this event was delivered by Fusion48 and gave the tools and resources for identifying frailty to be disseminated within organisations.
Her work on frailty has led to some national involvement including participating in workshops for developing an Older Persons Pathways for NHS Rightcare, Core Competencies for Frailty for NHS England/HEE and Skills for Health and development of an Advanced Practitioner role in frailty with HEE.
Locally she is part of NWAS Patient Priority Group for Older People led by Julie Butterworth and in March she hosted an Older Persons day (with help from Barbara Coleman) in Knutsford, with guest speakers including Fusion48’s Dawn Moody; the event received fantastic feedback. She delivers falls and frailty training to care homes as well as frailty awareness to local groups eg the Women’s Institute, lunch clubs. Her plan is to develop academically with frailty by incorporating it into my Advanced Practice Masters. She is married to a fellow paramedic and she has a cockapoo who is very spoilt!
